Dentist Salon is a patient intake tool operated by DentalDiagnostix. It allows dental offices to collect guided pre-visit information from their patients. By using Dentist Salon, you agree to these terms.

Dentist Salon is not a clinical service. It does not provide dental diagnoses, treatment recommendations, or medical advice of any kind.

You may use Dentist Salon if you are a patient who has been invited to complete an intake by a participating dental office. Use of this service by minors requires consent from a parent or legal guardian.

You agree to provide accurate and truthful responses. Dentist Salon is not responsible for outcomes resulting from incomplete or inaccurate intake information.

Completing an intake through Dentist Salon does not constitute a dental examination, diagnosis, or treatment. It does not replace direct communication with your dental office. If you have a dental emergency, contact your dental office or emergency services directly.

The answers you submit are shared with the dental office that invited you. By submitting your intake, you authorize that dental office to receive and retain your responses for the purpose of preparing for your visit.

You are responsible for the accuracy of what you submit. Do not include information that belongs to another person without their consent.

You agree not to:

• Submit false, misleading, or harmful information
• Attempt to access intake records belonging to other patients
• Use automated tools to interact with or scrape this service
• Interfere with the operation or security of the service

All content, design, and software comprising Dentist Salon is owned by DentalDiagnostix or its licensors. You may not copy, reproduce, or redistribute any part of the service without written permission.

Dentist Salon is provided "as is" without warranties of any kind. We do not warrant that the service will be uninterrupted, error-free, or suitable for any particular purpose.

To the fullest extent permitted by law, DentalDiagnostix shall not be liable for any indirect, incidental, or consequential damages arising from your use of this service.

We may update these terms as the service evolves. Continued use of Dentist Salon after changes are posted constitutes your acceptance of the revised terms. Material changes will be noted with an updated date at the top of this page.
